Supplements and OIs must be recertified in the anniversary month of their publication to remain current.
To recertify or rescind a supplement or OI, or to request an extension, please complete this form.
CAP/DA will post the recertification date. For questions about the publications process, or to request a Word version of anything posted here, email pubs@capnhq.gov.
Do not convert a Pdf to a Word file.
